One entry form covers one county. These are the counties the state listed for the 2026 drawing, with how many licenses are up in each. Which one you enter is your call, not ours.

Where the state writes to you

Every letter about this entry goes to this address, including a deficiency letter if the state finds a problem. You get 14 days to answer one of those.

Interested persons

The state asks for every interested person, and it says to include the entrant. Here is how the state describes who that covers. Read it and decide for yourself. We cannot decide this one for you.

A person interested in the license includes, but is not limited to, any person who has: a financial arrangement through joint funds with the entrant; an investment in the entrant's business or acquisition of the license; an obligation as a co-signor or guarantor of a note or lease related to the entrant's business or the acquisition of the license; taken any action that creates funds to enable a transaction or the operation of the entrant's business. Interested persons may include domestic partners, spouses, officers, directors, stockholders, limited and general partners, corporations or other legal entities, or any other entity having an interest in the license to be issued or in the legal entity for which the entry is filed.

The form prints four rows. Add more if you need them and we will attach a continuation page, which the form allows.

Who signs

For a personal entry, every person you listed above signs. Not one of them, all of them. Signatures go on in ink after you print it. If somebody signs for another person, a notarized Power of Attorney goes in the envelope.
