See exactly what you get
Most people buying this have never seen a finished application. So here is a whole one, start to finish, before you spend anything.
These are not mockups. Every file below came out of the same generator a paying order goes through. The customer is invented, the business is called Example Bar LLC, and every page is stamped SAMPLE so nobody tries to file it.
This is a Beer & Wine packet, $249. Eleven files. A full liquor restaurant order looks the same with two extra pages and one more sheet per owner.
Start with the cover sheet
This is the page that decides whether the rest is any use. It lists every box we deliberately left empty, every signature, and every agency sign off, so nothing gets missed on the way to the counter.
We never collect Social Security numbers, passport numbers, or immigration card numbers. The state's form asks for them. We do not, so a breach here cannot cost you your identity. The cover sheet tells you which page and which line to fill in yourself.
What else is in the packet
- Section 3, personal informationOne of these for every person with a stake in the business. Filled in, except the Social Security box, which we never collect.
- Section 4, the premisesPre-headed, with the floor plan box left empty for your drawing.
- Section 5, the approvals pageThe page you carry to zoning, to the Department of Revenue, and to the health authority.
- Section 6, the business history questionFilled from your answer.
- Section 9, the affidavitReady for you to sign in front of a notary.
- Portal answer sheetEvery answer, in the order the online system asks for them, with both the menu name and the page title so you can tell where you are.
- Filing roadmapWhat to do and in what order, for your county. Fingerprints, Revenue, zoning, health, notary, filing.
- Document checklistBuilt from your own answers. Say yes to a history question and the document the state wants appears on your list.
- Fee statementWhat the state charges you, with the arithmetic shown so you can check it.
- The whole ABT-6001All nine pages, for filing on paper instead of online.
What you still do yourself
We are a document preparation service, so the honest answer is: more than a lawyer would leave you, and that is why this costs a few hundred dollars instead of a few thousand.
- You sign it, and get two pages notarized.
- You get fingerprinted in person, at least 5 days before you file.
- You collect three sign offs: zoning, Revenue, and health.
- You draw the floor plan.
- You file it and you pay the state.
The roadmap in the packet says who to go to, in what order, and what catches people out at each step.
